This works perfectly. The only challenge I have is the click noise. Seems noisy for small relays but at least you know it works. There is an in-line LED on each channel, so you can see and test easily.
The 12V source is needed to drive the relay coils. The relay contacts can be 120V ac, 12V dc, just about anything. perfect for christmas lights or other. NO and NC contacts on the relays, and a built in fly back diode.
